NJ FamilyCare Housing Supports Program

Serving Atlantic, Burlington, Camden, Cumberland, Gloucester Counties

The NJ FamilyCare Housing Supports Program is a new initiative to help individuals with Medicaid and insecure housing to find and keep housing – because housing is a crucial part of overall health.
Eligible Medicaid members can receive assistance finding, moving into, and maintaining housing, as well as repairing or modifying their home to ensure their health and safety. 

We provide tenancy sustaining and move-in supports only.

What are tenancy sustaining services?

  • Examples include: Educating you about your rights as a tenant, assisting with voucher recertification, applying for utility assistance, supporting development of independent living skills to help you stay housed, helping you access social programs, connecting you with transportation, childcare resources, and other community services.

What are move-in supports? â€‹

  • Examples include: Payment for non-recurring, one-time transitional expenses provided to a beneficiary during the transition period to their own home, including: Payment for the set-up of the new housing unit, to address needs identified in the person centered care plan; services required for a beneficiary’s health and safety, such as pest eradication and one-time cleaning prior to move-in; Purchase of household furnishings needed to establish community-based tenancy including furniture, food preparation items, pantry stocking, or bed/bath linens. If necessary, assistance may also be provided to help set up these items; Payment for items to support the details of the move, as appropriate, including:  Costs for filing applications and payment of application fees necessary to obtain a lease on a home; Payment for move-in costs including movers to ensure transportation of self and possessions to new housing arrangement; Payment of security deposits; Payment of set-up fees or deposits for utility or service access, including telephone, electricity, heating and water.

Am I eligible?

  • Eligible NJ residents must have Medicaid and meet at least one social-risk criteria (homeless, at-risk of homelessness, transitioning from an institution, or recently released from correctional facilities), and Meet at least one clinical-risk criteria (such as: chronic health condition, mental health condition, substance misuse, pregnancy, complex medical health due to disability, sexual / domestic violence, assisted living needs, repeated emergency use /hospitalization).


How much does it cost?

  • This service costs you nothing. It is covered by Medicaid for eligible individuals.
