Part Time Support Coordinator
Hunterdon, Warren, and Sussex Counties
As a Professional Support Coordinator (SC), you will provide case management services to individuals with intellectual and/or developmental disabilities.
The goal is to work together to create an individualized plan that incorporates all aspects of care. The SC is also responsible for maintaining on going monitoring to provide exceptional care.
The ideal candidate believes in the abilities and teachability of all people. They are professional, quick to respond and forward thinking. Accurate and timely documentation is a must.
At least a bachelor's degree from an accredited institution (required)
At least 1 year of experience as a Support Coordinator. (preferred)
At least 1 year of experience working with individuals with intellectual disabilities from a professional, personal and/or volunteer basis.
Completion of NJ DDD Boggs Center required SC orientation
A dedication and commitment to individuals with developmental disabilities.
You must possess:
Willingness to work a flexible schedule, as needed, to meet the needs of the individuals
Willingness to work from home to complete documentation
Possession of a valid driver's license and a fully registered and insured vehicle. SCs must use own vehicle are not currently reimbursed for mileage.
Able to travel throughout Central and South Jersey
Ability to pass pre-employment background checks
Part Time Support Broker
Middlesex and Monmouth Counties
This contracted position supports people to direct their own services through the provision of support broker services to people with intellectual disabilities who are using the Self Directed Model. The Support Broker provides information; guidance and technical assistance to the individual so that s/he can access, organize and utilize opportunities, resources and services necessary to live a self determined life. Services are designed to provide assistance as needed with employer-related functions, planning, accessing community resources.
• Be at least 18 years of age.
• One-year experience in a management position with human resources responsibilities OR have a degree in Human Resources.
• Possession of a valid driver’s license with a safe driving record AND a reliable means of transportation.
You must possess these abilities:
Work effectively as a member of a team
Communicate effectively both orally and in writing
Working knowledge and application of basic employment law, principles of self determination, person centered thinking and planning
Use the training offered to meet the unique needs of the individual supported including her/his behavioral, communication and mobility needs.
