Ability Advocates of New Jersey is hiring Part Time Support Coordinators to serve consumers supported by the New Jersey Division of Developmental Disabilities. This is a work from home/on the road job, with the opportunity to utilize our office space in Stratford, NJ when needed. Prospective Support Coordinators will serve participants in either the North, Central, South, or Shore areas.

Responsibilities:​

  • Working with the person and family in creating a support and service plan that details: how best to support the person, defines the needed services and their cost while ensuring the plan is within their budget, selects providers that will implement the plan

  • Monitoring the implementation of the plan with monthly phone contact, quarterly face-to-face visits, annual home visits, and submitting monitoring tools by the 24th of each month

  • Adjusting the plan as needed to reflect the changing goals and needs of the consumer and or family

  • Assisting the person and family in successfully navigating the complex social service and provider system supporting people with disabilities

  • Effectively listen, communicate, and provide compassionate support to the consumer and their family

  • Responding within 24 hours to all requests for information from consumers, their family, providers, DDD, and supervisors regarding the consumers you serve

  • Effectively use technology in communication with consumers and preparing service plans

  • Represent Ability Advocates of New Jersey in a professional manner

Requirements:

  • Bachelor's degree in any field required

  • One year of direct experience with people with developmental or intellectual disabilities including professional, volunteer, and family experience

  • One or more year as a Support Coordinator under DDD

  • Must be able to pass all state and federal background checks

  • Must be willing to build caseload gradually to at least 10 individuals

  • Must own a computer and have access to secure internet

  • Must be willing to travel to and around New Jersey

  • Must have reliable transportation

  • Strong organizational and communication skills

  • Proficiency in Microsoft Office

  • Ability to manage multiple tasks and priorities effectively​​
